New Patient Forms
The New Patient Packet is an online form and can be completed securely without having to print, download, and bring physical forms into the office.
The New Patient Packet should be completed and submitted before your first appointment is scheduled.
The HIPAA Authorization form and Records Release can be downloaded and printed for you to bring in with your first visit.
Billing and Insurance
Horizons Diagnostics accepts most major insurance policies. If you have a question about your insurance coverage, please call our office at 706-321-0476. Prior to your arrival, please be prepared to provide a copy of your insurance card and if applicable, we will collect your co-pay prior to your visit.
